USAble Life offers a competitive employee benefits package that includes:
INSURANCE PLANS
- Health Insurance A PPO (Preferred Provider) plan through administered by Blue Advantage Administrators of Arkansas is provided for all employees. Individual coverage is provided at no cost to employees. Family coverage requires a small employee contribution. The effective date of coverage is the first of the month following your hire date.
- Dental Insurance The Company provides individual dental coverage at no cost to the employee. Family coverage requires an employee contribution. The effective date of your coverage is the first day of the month following your hire date.
- Vision Insurance Employee and family coverage require an employee contribution. The effective date of your coverage is the first day of the month following your hire date.
- Life Insurance Group Basic Life and Accidental Death & Dismemberment insurance is provided by the Company at no cost to the employee. The amount of coverage is 2 times annual salary up to a maximum guaranteed issue of $500,000.
- Supplemental Life Supplemental Life insurance is available to employees at 1x, 2x or 3x salary. There is an $800,000 combined maximum for basic and supplemental life insurance.
- Dependent Life Dependent Life insurance is available for the eligible dependents of the employee. Eligible dependents of the employee are the employee's spouse and unmarried children, step-children or adopted children age 14 days to 19 years (age 23 for full-time students).
- Voluntary Accidental Death & Dismemberment(VAD&D) VAD&D is additional 24-hour AD&D insurance the employee may purchase. Family coverage is available.
- Short Term Disability Employees are eligible after one year of service. STD is provided by the Company at no cost to you.
- Long Term Disability Employees are eligible after one year of service. LTD Option 1 is provided by the Company at no cost to you. Option 2 is a buy-up plan paid for by the employee.
- Universal Life, Cancer Care Elite, Cardiac Care and Accident Guard and Long Term Care policies can be purchased on a payroll deduction basis.

- 401(k) Savings Plan USAble Life offers an easy way for employees to save - the employee 401(k) Savings Plan. The 401(k) Plan provides you with a way to save money, invest your savings and defer federal and state income taxes. Best of all, company contributions give you incentive to save more and watch your savings account grow. All employees are immediately eligible to participate in the plan. Your match is deposited into your account every two weeks and is 100% vested immediately.
