Group Term
Life/AD&D/Dependent Life
Group Term Life, Accidental Death and Dismemberment (AD&D),
and Dependent Life coverage is designed to provide benefits
to the beneficiary for loss of life, or to the insured for
loss resulting from accidental dismemberment.
The benefits are provided through a group master policy issued
to and purchased by the employer on behalf of all its employees.

| Minimum Participation |
Non-contributory: 100% if the employer
pays the entire cost. Contributory: 75% if the employees
contribute to cost. |

Choose amounts from $10,000 to a maximum
of $500,000. |

Suggested benefit reduction:
35% at age 65, 50% at age 70 and termination at retirement.
(Other schedules may be quoted upon request)
Accidental Death & Dismemberment will reduce the same
as life, except coverage must terminate at retirement.

Spouse: $2,000 or $5,000
Children Age 14 days to 6 months: $100 or $200 Children
Age 6 months to 19 years: $1,000 or $2,000

| Conversion Privilege |
When the employee's insurance terminates
because of death, retirement or termination of employment,
dependent coverage may be converted under the same conditions
outlined for conversion of the employee's group life insurance.
In addition, any dependent no longer eligible, under the
terms and provisions of the master policy may convert
his coverage. |

Group
Short Term Disability
Group Short Term Disability (STD) is designed to provide
partial income replacement for employees who become disabled
as the result of sickness or an off-the-job injury. Benefits
are payable according to a schedule for the benefit periods
as shown in the master policy.

| Minimum Participation |
Non-contributory: 100% if the employer
pays the entire cost. Contributory: 75% if the employees
contribute to cost. |

13 Weeks or 26 Weeks (Other benefit periods
may be available upon request to coincide with a Long
Term Disability Plan's elimination period.) |
| Benefits Begin |
The 1st, 8th, 15th, or 31st day of disability
for accident and/or sickness. First day benefits, for
accident only, are also available. |

Maximum percentage is 70% of basic earnings to a maximum

Earnings Schedule with STD benefits equal
to 50%, 60%, 66 2/3%, or 70% of the employee's basic weekly
income to a maximum dollar amount
Occupational Schedule with classes and amounts that must
be designed so that the amount of insurance will not exceed
70% of the lowest earnings in each class. |

Long Term Disability
Group Long Term Disability (LTD) is designed to provide partial
income for employees who become totally disabled due to sickness
or injury occurring on or off-the-job. Benefits are payable
according to a schedule for the benefit periods as shown in
the master policy.

Non-contributory: 100% if the employer
pays the entire cost.
Contributory: 75% if the employees contribute to cost. |

Recommended: 60%
Options: 50%, 66 2/3% |
| Maximum Monthly Benefit |
Standard maximum: $6,000.
The maximum monthly benefit will be based on the average
monthly salaries of the five highest paid employees. |

Recommended: 90 or 180 days.
Options: 120, 150, 360 days |
