Group Term Life
Voluntary Group
Term Life (VGTL) is available to individuals whose employer
has sponsored the plan and has provided for premium payments
to be made through payroll deduction. A group contract is issued to the sponsoring employer.
| Eligibility |
• Actively at work, at least 20 hours per week for your employer;
• Under age 70 on the effective date of your coverage; and
• Satisfy the waiting period set by your employer, of not less than 30 days. |
| Employee Amount |
Elected in increments of $10,000 to a maximum
of $300,000. |
| Dependent Coverage |
Spouse - you may purchase coverage in $10,000 increments to a maximum of $300,000.
*Children - you may purchase coverage for either $5,000 or $10,000 for all eligible dependent children.
*Coverage amounts
may be limited by the laws of the state of issue |
| Accelerated Benefit |
This benefit applies to those employees who have a terminal illness, as defined in the terms and provisions of the group policy. The benefit allows a covered person to receive part of his or her insurance while still living. The amount payable to the insured can be as much as 50% of the life insurance amount. |
| Waiver of Premium |
If you terminate your employment before age 60 because of your total disability, which continues for at least 6 months during which time premiums are paid, your life insurance may be continued while you are totally disabled without payment of premiums to age 65. |
| Reductions/Termination |
If you are still actively at work on a full-time basis, VGTL benefits for you reduce 331/3% of the pre-age 65 amount at ages 65 and 70 and terminate at retirement. Spouse coverage terminates on the earlier of the date your spouse is no longer eligible for coverage, the date he reaches age 65, or the date your employment with the group policyholder terminates. VGTL coverage for eligible dependents terminates on the earlier of the date they are no longer an eligible dependent or the date your employment with the group policyholder terminates, whichever occurs first. |
| Continuation of Insurance |
You may continue your and your spouse’s coverage if your employer has chosen the portability benefit option and if on the date your employment terminates:
• you have been covered for a minimum of 12 months; and
• you are not disabled; and
• you are either:
a) not retired; or
b) are retired and are under age 65.
Dependent children are not eligible for the portability benefit option, but their coverage maybe converted to an individual policy when they are no longer eligible for coverage. |
| Termination |
Ported coverage will terminate if premiums
are not paid within 30 days of due date, on the date the
Group Trust Policy terminates, or when the insured reaches
age 70. Conversion is available. |

Voluntary Accidental Death & Dismemberment
Voluntary Accidental Death & Dismemberment (VAD&D) allows you to select accidental death and dismemberment benefits that protect your income in the event you suffer an unexpected loss due to accidental death or bodily injury. VAD&D provides full 24-hour protection against accidents anywhere in the world and is available to individuals whose employer has sponsored the plan and has provided for premium payments to be made through payroll deduction. A group contract is issued to the sponsoring employer.
| Eligibility |
• Actively at work, at least 20 hours per week for your employer;
• Under age 70 on the effective date of your coverage; and
• Satisfy the waiting period set by your employer, of not less than 30 days. |

| Covered Losses |
• Loss of life - 100%
• Loss of two or more members - 100%
• Loss of one member - 50% of the loss of life benefit
• Loss of thumb & index finger (same hand) - 25% of the loss of life benefit
If the insured person sustains more than one of the above losses as the result of any one accident, we will pay only the one largest amount to which the insured person is entitled.
A member means hand, foot, eyesight, speech, or hearing. |
| Additional Benefits |
• Seat Belt Accidental Death - 10% of loss of life benefit, not to exceed $25,000
• Air Bag Accidental Death - 10% of loss of life benefit, not to exceed $10,000
• Special Education Benefit for Spouse and Children - up to $2,500 for 4 years

Voluntary
Income Protection
Voluntary Income Protection (VIP) allows you to select short term disability benefits that protect your ability to earn an income in the event you suffer an illness or accidental injury. If you lose the ability to earn a paycheck, VIP will give you the peace of mind that you will be able to continue making payments on your home and giving your family financial stability and is available to individuals whose employer has sponsored the plan and has provided for premium payments to be made through payroll deduction. A group contract is issued to the sponsoring employer.
| Eligibility |
• Actively at work, at least 20 hours per week for your employer;
• Under age 70 on the effective date of your coverage; and
• Satisfy the waiting period set by your employer, of not less than 30 days. |
| Amount |
You may elect your weekly benefit in $10 increments, not to exceed the percentage of your weekly earnings, as selected by your employer. The plan pays the weekly benefit, up to $750, directly to you in the event you are totally disabled from a sickness or accidental injury. |
| Definitions |
Total Disability or Totally Disabled
• You are completely unable to perform all of the material duties of your own regular occupation at your customary place of work; and
• You are not actively working in any occupation for pay or profit; and
• You are under the regular care of a physician.
Partial disability
As a result of the sickness or injury which caused total disability:
• You are able to perform one or more, but not all, of the material and substantial duties of your own or any other occupation on a full-time basis; or
• You are able to perform all of the material and substantial duties of your own or any other occupation on a part-time basis.
Pre-existing condition
A diagnosed sickness or injury for which you received treatment within twelve (12) months prior to your effective date or the effective date of an increase in coverage. Treatment means consultation, care, or services provided by a physician, including diagnostic measures and taking prescribed drugs and medicine. |
| Partial Disability |
You may receive up to 100% of your pre-disability income if:
• Partial disability begins within 31 days of the date total disability ceases.
• You are earning less than 80% of your pre-disability income at the time the partial disability employment begins.The partial disability benefit is the lesser of:
• The weekly benefit selected; or
• 100% of your pre-disability earnings, less your partial disability earnings. |
| Reductions |
If you are still actively at work on a full-time basis, VIP benefits reduce 331/3%when you reach age65 and will terminate at age 70 or your retirement, whichever occurs first. |
| Waiver of Premium |
If you are totally disabled for 90 consecutive days, future premium payments that fall due will be waived as long as you are receiving benefits. |
| Pre-Existing Exclusion |
The plan does not pay for disabilities caused by or related to a pre-existing condition during the first year of coverage, or an increase in coverage. |

Long Term Disability
Voluntary Long Term
Disability (VLTD) is long term disability insurance, which
is available to individuals whose employer has sponsored the
plan and has provided for premium payments to be made through
payroll deduction. Monthly benefits help replace lost income
in the event of the insured’s total disability, including
maternity. A group contract is issued to the sponsoring employer.

All full-time employees working
at least 20 hours per week. |
| Minimum Size Group |
10 lives or 15%, whichever is greater. |
| Monthly Benefit Amount |
Up to 60% of basic monthly earnings.
Benefit may be selected from $200, in increments of $100
to a
maximum of $5000. |
| Choice of Plans |
The employer selects one of three plans
with either 90 or 180 day elimination period. Plan
1 Maximum benefit duration of 5 Years Accident &
2 years Sickness. Plan 2 Maximum benefit
duration of 5 Years Accident & Sickness . Plan
3 To Age 65 benefit duration for Accident and Sickness.
Rates are based on the group's
industry. |
| Guaranteed Issue |
Amounts are Guaranteed Issue up to the
maximum of $5000/month. No evidence of insurability is
required, except for late applicants and increases in
coverage. |
| Definition of Disability |
24 month "own occupation." Thereafter,
"any occupation." |
| Waiver of Premium |
Future premium payments are waived as long
as disability benefits are being paid. |

GAPCare
GAPCare is a voluntary
group insurance plan sponsored by the employer, which provides
medical expense benefits that are not paid by a group health
insurance plan. Eligible employees must be covered under their
employer's or their spouse's employer's group medical plan.
GAPCare supplemental insurance provides extra protection to
"bridge the gap" between medical care expenses and
benefits not paid by the group medical plan of the employee
or the employee's spouse.
Coverage is available to employees and their families. The
employer chooses from several Inpatient Hospital benefits
and deductibles. Coverage also includes both Outpatient Hospital
and Physician Outpatient Treatment benefits. Plus, an optional
Wellness benefit is available.
