CancerCare Elite provides supplemental coverage when certain losses occur as a result of cancer or a covered specified disease. CancerCare Elite pays directly to the insured, regardless of other coverage. Coverage is available to employees and their families, with a choice of Plan I, Plan II, or Plan III.

| Cancer Diagnosis |
Pays on first diagnosis of internal cancer. Options available are: $1,000, $2,000, $3,000, $4,000 and $5,000. Insured family members qualify for 100% of the benefit.

| Hospital Intensive Care |
Pays daily benefit when injuries or accident results in confinement in a Coronary Care or Intensive Care Unit. Daily benefit options available are: $200, $400 and $600.

| Cancer Disability |
Pays monthly income elected up to one year if disabled as result of internal cancer. Family coverage may also selected if the spouse actively works at least 20 hours per week. Children are not eligible. Rider terminates at insured’s age 65. Monthly benefit options are: $250 and $500.
