CancerCare Elite

CancerCare Elite provides supplemental coverage when certain losses occur as a result of cancer or a covered specified disease. CancerCare Elite pays directly to the insured, regardless of other coverage. Coverage is available to employees and their families, with a choice of Plan I, Plan II, or Plan III.

Eligibility

Ages 18 to 64.


Guaranteed Renewable

Guaranteed renewable for your lifetime.


Inpatient Benefits

Plan I   $100 daily - first 60 days; $200 each subsequent day.
Plan II  $250 daily - first 60 days; $500 each subsequent day.
Plan III $300 daily - first 60 days; $600 each subsequent day.


Inpatient or Outpatient Radiation Chemotherapy and Blood and Plasma

Plan I $5,000 maximum per calendar year.
Plan II $10,000 maximum per calendar year.
Plan III $15,000 maximum per calendar year.


Inpatient or Outpatient Surgery and Anesthesia

Plan I   $1,000 maximum per operation.

Plan II  $2,000 maximum per operation.

Plan III $4,000 maximum per operation.

Anesthesia pays 30% of the amount payable under the surgical benefit.

Wellness Benefit – pays up to $75 per calendar year for the following screening tests:

    • Mammography
    • Thermography
    • Colonoscopy
    • Hemocult Stool Analysis
    • PSA (blood test for prostate cancer)
    • CA125 (blood test for ovarian cancer)
    • CEA (blood test for colon cancer)
    • Pap Smear
    • Flexible Sigmoidoscopy
    • Chest X-Ray
    • Any diagnostic procedure which can lead to the positive diagnosis of cancer.


Cancer Diagnosis

Pays on first diagnosis of internal cancer. Options available are: $1,000, $2,000, $3,000, $4,000 and $5,000. Insured family members qualify for 100% of the benefit.


Hospital Intensive Care

Pays daily benefit when injuries or accident results in confinement in a Coronary Care or Intensive Care Unit. Daily benefit options available are: $200, $400 and $600.


Cancer Disability

Pays monthly income elected up to one year if disabled as result of internal cancer. Family coverage may also selected if the spouse actively works at least 20 hours per week. Children are not eligible. Rider terminates at insured’s age 65. Monthly benefit options are: $250 and $500.
